ANN ARBOR, Michigan, July 30, 2015 – The Consortium of Independent Physician Associations’ (CIPA) rigorous efforts to guide physician practices to provide exemplary quality care and lower costs generated 212 Patient-Centered Medical Home (PCMH) designations by Blue Cross Blue Shield of Michigan (BCBSM).  The designation period runs from July 1, 2015 through June 30, 2016.

Throughout the past year, CIPA, managed by Medical Advantage, enhanced its team’s consulting efforts into a driving force behind population health management and performance improvement at the practice level.  The result was an increase in primary care physicians in Michigan who are improving healthcare quality by adopting the PCMH model of care.

“Our CIPA team was able to improve quality and the patient experience through organizing primary care offices into a supportive, patient-centered infrastructure,” said Linda Mackensen, director of CIPA field operations. “As a result of the CIPA teams’ guidance, practices improved the health of their patient population, while providing high-quality, low-cost care.”

In a PCMH, the patient’s physician (primary care, internist or pediatrician) leads the care team.  All of the patient’s health records (including visits with specialists, test results and prescriptions ordered by other physicians) are maintained and tracked centrally within the PCMH.  Patients have 24-hour access to their PCMH, a benefit that can keep patients at home and out of the emergency room.

Established in 2005, CIPA is a Michigan-wide provider network consisting of approximately 40 independent physician associations (IPAs), physician hospital organizations (PHOs), federally qualified health centers, rural health centers, individual physicians, and group practices representing more than 1,400 physicians.
