EAST LANSING, Michigan, September 13, 2016 – Medical Advantage announces its largest physician organizations, Consortium of Independent Physician Associations (CIPA) and Professional Medical Corporation, have a total of 176 primary care practices recently designated by Blue Cross Blue Shield of Michigan (BCBSM) as Patient-Centered Medical Homes (PCMHs). The total number of designated PCMHs in 2016 represents a 13% increase over last year’s designations.
BCBSM’s PCMH program focuses on the health needs of each patient and coordinates patient care across all settings. The PCMH program recognizes and rewards primary care physicians who have made significant progress implementing patient-centered capabilities in their practice. PCMH practices often have services available to patients that non-designated practices don’t offer such as 24-hour access to the care team. PCMH practices also coordinate care with other healthcare providers in the community for nutrition counseling, home care, and other services. And, they teach patients how to manage conditions such as asthma and diabetes.
Medical Advantage’s practice transformation team works collaboratively with hundreds of practices throughout Michigan to help them achieve PCMH status. The team meets onsite at the practice to implement various capabilities in order to become nominated and designated as a PCMH. To date, Medical Advantage has assisted in over 1,000 practice transformations since 2009.
